Sword Health Enters Portugal's Public Health System as a Prescribable MSK Therapy

Sword Health has signed a partnership with Portugal's National Health Service (SNS) to make its AI-driven remote physiotherapy platform available to public system patients. Physicians in Portugal can now formally prescribe Sword's program for common musculoskeletal conditions including lower back pain, knee pain, and shoulder pain.

Why it matters

Sword Health's agreement with Portugal's SNS is not a pilot, a research collaboration, or a wellness benefit. It is a formal prescribing pathway inside a national public health system. That distinction matters. When a physician can write a prescription for a digital platform the same way they would refer to outpatient physiotherapy, the product moves from optional to infrastructural. That is a different kind of commercial and clinical relationship.

The conditions covered, lower back pain, shoulder pain, knee pain, sprains, and muscle strains, represent an enormous share of primary care and outpatient physiotherapy volume in any health system. If Sword can demonstrate equivalent or better outcomes at lower cost in a public system context, the pressure on traditional physiotherapy delivery models will intensify. If it cannot, this will become a cautionary case study in how vendor success in employer markets does not automatically translate to the complexity and heterogeneity of national health populations. Either way, the data that comes out of this partnership will be worth watching closely.

What to watch

Whether SNS publishes outcomes data from this program and whether other European public health systems use this as a template for contracting AI-enabled allied health services.
